The policy of safety unit in Iranian hospital

According to the World Health Organization, patient safety is the prevention or reduction of the probability of health care-related harm and injury to the lowest acceptable level.
Iranian Psychiatric Specialized Hospital with the motto "Our hospital is patient safety friendly" is trying to improve safety and reduce errors. Providing medical services to patients, continuously improving the knowledge, skills and performance of employees and reducing errors is the main policy of the hospital and patient safety, which in order to Achieving this goal includes the following programs
Conducting visits, interacting with personnel, brainstorming sessions for better performance in order to improve safety, correct reporting of 28 events

Management visits

During management visits conducted with senior managers (hospital director, nursing director, quality improvement director), along with the patient safety coordinator, participating individuals introduce themselves to the target staff. The purpose of these visits, which cover various departments and units, is to listen to the issues from the staff members' perspectives. Additionally, suggestions from the staff are utilized to enhance patient safety and satisfaction.

Field visit

During field visits, the educational supervisor, environmental health specialist, occupational health officer, and patient safety coordinator accompany each other. Each of these officials provides necessary training to the staff for achieving patient safety based on the checklists they have. Feedback on the findings is then provided to the relevant department and personnel

Error reporting

Cultivating patient safety and error reporting is one of the main goals, and this reporting and root analysis of the error makes it possible to learn from the errors that have occurred and prevent the repetition of the error, as well as report the medication error and deal with it. Staff training

Employee training policies

The Iranian Psychiatric Hospital for Iranians, in line with accreditation requirements aimed at enhancing the quality of healthcare services, continuous improvement of staff knowledge, skills, and performance, as well as reducing errors, implements a four-stage educational policy for both new recruits and existing staff.

Training of new personnel

After new staff members commence their employment and undergo an initial competency assessment by the unit's head nurse or experienced nurse, their educational needs are identified. These needs are then referred to the education department for further completion of training. Following the training, an evaluation is conducted, and before being introduced to the hiring committee, they participate in an entrance exam. If they achieve the required score, they are recommended to the hiring committee.

Given that the most fundamental step in developing and implementing educational programs is the proper execution of needs assessment, staff nursing needs are prioritized based on individual development plans declared by nurses and nurse supervisors, as well as in alignment with overarching educational policies aimed at patient safety, accreditation standards, and patient rights charter, within a prioritized framework.

Internal departmental training, face-to-face training, organization of in-house training courses, introduction of personnel for participation in external training courses, seminars, conferences, and webinars, internal and face-to-face training sessions are planned, executed, and evaluated by the head nurse of each department in collaboration with the department's training liaison officer based on the needs of the respective department. Performance reports are then submitted to the training department.

  • Annual educational needs assessment for patients and their companions in all departments is determined based on their level of knowledge and awareness, as well as the nature of each department.
  • The collection of educational needs for each group is carried out by the hospital's educational supervisor.

Educational priorities

  • The prioritization of educational needs for patients and their companions is conducted with the presence of the educational supervisor, health education supervisor, nurse manager, and the nurse responsible for patient education in the department.
  • Planning to address educational needs is carried out based on reputable and standardized education.
  • Following prioritization, educational needs that patients require the most are incorporated into the educational program. If these needs are met, other educational topics are then addressed.

Patient education methods and measures

  • Teaching methods to the patient include: face-to-face teaching, using educational pamphlets and simple educational booklets, showing educational videos.

Monitoring the implementation of patient education programs

  • Monitoring the implementation of patient education programs in each department based on regular audits and interviews with patients and their companions.
  • Monthly evaluation of the effectiveness of education provided to patients by the educational supervisor and the nurse manager responsible for patient education.
  • Supervision of the effectiveness assessment of education provided to patients on a quarterly basis by the educational supervisor.
  • Presentation of the results of the effectiveness of education to the departments.
  • Implementation of corrective actions outlined and reevaluation of the effectiveness of education.
